OLW Phone Directories:
The FSA's handbook/phone directories were sent home with oldest/only students. If you did not receive your pre-ordered directory(ies) or if you have any questions please call Karen DeZellar at 577-5202. If you did not order a directory and/or need additional copies they are available for sale ($5) in the school office or at the school store on Tuesdays.
Dental and Physical Forms for the '08-'09 School Year:
Dental Forms for all students were sent home March 10th. The state recommends students visit the dentist every six months but they require that students in grades Kindergarten, Second, and Sixth submit a completed dental form before the First day of the new school year. Dental forms for students entering K, 2, & 6 should be turned in to the office BEFORE the first day of school in the fall. All of the other grade levels should turn in the dental forms at least once a year.
Physical forms for students entering Kindergarten or Fifth Grade were also sent home Mar. 10. The state requires all new students entering school for the first time and those entering Kindergarten and and Fifth grade to have a complete physical with updated immunizations. These forms should be completed and turned into the office BEFORE the first day of school in the fall.
**NOTE**: The Illinois State Board of Education has changed the requirement for physical/immunizations from 5th grade to 6th grade, therefore, current 5th graders are "grandfathered" into the 6th grade requirement, and current 4th graders won't be required to have a physical until 2009-2010 school year.

As more and more children use the Internet to communicate with friends, their safety is a source of concern to all of us here at Wayside School. We have provided the following links and documents so that parents can act in advance to keep their kids safe when they use the Internet to communicate.
